Adult Services

Adult Services Units:

Adult Mental Health: Assists clients with mental illness by providing services including case management, referrals to community mental health services and outreach programs, employment services, and coordination with residential treatment facilities. 

Adult Protection Services: works to protect our counties most vulnerable adults from abuse, neglect and financial exploitation, giving respect to individual choice and self-determination.  To report suspected abuse of a vulnerable adult call 1-844-880-1574 

Substance Use Disorder (SUD): provides funding for those who are uninsured/underinsured and seeking SUD treatment. Area resources for where a comprehensive use assessment can be completed and list of facilities where treatment could be sought are available upon request.  

 

Rule 185 Case Management: provides services to those with developmental disabilities or related conditions. The intent of the program is to strengthen the person's self-reliance and provide community support services while protecting clients from abuse, neglect, or exploitation. Services available include case management, Semi-Independent Living Services (SILS), respite care, supported employment, guardianship, and referrals for other necessary services. 

MnCHOICES Assessments: Assessment process designed to screen an individual’s eligibility for home and community-based services, PCA services and level of care for nursing home placement. 

Disability and Aging Waiver Programs: Arrange for and monitor home and community-based services for disabled adults and children, and for seniors 65 years of age and older. Programs available are Elderly Waiver (EW), Alternative Care (AC), Community Alternative Care (CAC), Community Alternatives for Disabled Individuals (CADI), Brain Injury (BI) and Developmental Disabilities (DD) Waivers. These programs are intended to promote community living and independence by providing the appropriate health care, home care, and support services based on individual needs.
